Paperwork Turn In Night NEEDED for: ALL Football & Cheer -
PAPERWORK NEEDED BEFORE ATHLETE CAN STEP ONTO THE PRACTICE FIELD
1. Original Birth Certificate or original certified abstract of Birth Certificate
2. Completed PYFL Physical Form
3. Proof of Residency that matches the address on the sign up contract (must be an ORIGINAL, no copies)
4. Small wallet size photo of player (football or cheer)
Birth Certificate and Utility Bill will be returned AFTER Certification Day later in August.

Physicals a doctor will be doing PYFL Physicals if you still need to get one for your athlete from 5:00-7:30 on July 26th at John Muir Middle School. $30 per physical.
